Job Description:
The Primary Care Case Manager provides case management &
coordination services to clients with multiple chronic conditions
and social stressors. The case manager is a part of a collaborative
team working to provide stability & resources to clients living
with a variety of complex physical health circumstances. The case
manager facilitates communication between the patient, primary care
team, behavioral team and other CommuniCare departments and outside
agencies. CommuniCare Health Centers is a Federally Qualified
Health Center providing comprehensive patient-centered primary care
services including medical, dental, substance abuse treatment,
behavioral health and outreach services to the low income,
multi-ethnic, uninsured and underinsured in the communities we
serve.
SCHEDULE: Full-time (40hr/week, some evening hours required) in
West Sacramento, California. Some travel in Yolo County will be
required.

Provide care coordination to clients with multiple medical needs
and/or those determined to be at high risk for complications.
Knowledge/Experience: Experience working in a social service,
health related organization or medical office/clinic. Excellent
interpersonal, oral and written communication, and presentation
skills. Strong organizational skills and the ability to multitask.
Demonstrated ability to assume responsibility and to work
independently as well as in a team. Ability to work in a busy
environment. Knowledge of medical terminology strongly preferred. A
service orientation characterized by cultural sensitivity and
polite communication with patients and staff members.
Special Skills/Equipment: Must provide proof of current driver's
license & auto insurance.
Physical Requirements and Work Environment: *The physical demands

operate office equipment, including computer equipment and phones.
Hearing required. Ability to see colors and to bend and move arms
in sufficient range of motion to complete patient interactions and
train staff. Regular travel to all agency sites required.
COMPLETED COVID-19 VACCINATION REQUIRED
SALARY/BENEFITS:Compensation depends on experience. Medical,
